We Will Dscuss a Costly Human Factors Failure Often when there is an aircraft accident, we quickly look for obvious casual factors. Such as a broken aircraft component, or a bad weather issue, or maybe pilot error. It is not uncommon to stop looking for the causal factor when one of the above appears to be the issue. Where there human factor signs that should/could have been recognized that may have prevented the accident? Good for one hour of IA Renewal Training.
