Pilots can often identify an individual risk to a flight and then mitigate it through avoidance, training, decision making, or some other measure. But risks don t always come one at a time. When multiple risks assail a pilot, we might consider these to be risk stacking, and the complex result may be difficult or impossible to fully mitigate. For this scenario accident which unfortunately ends in tragedy, we can analyze the initial risk and the consequences of additional risks which were stacked on top of the fist one. Let s explore some lessons learned from this accident and determine what we can apply to our individual flights to make them ever safer. This safety seminar is presented by the AOPA. The speaker is Ron Timmermans. Ron is an Orlando-based CFI and ATP rated pilot who has spoken at many aviation events. He is a Master CFI who was awarded the FAA Wright Brothers Master Pilot Award.
